The Big Problem:Approximately 1.7 million older adults received care in over 16,000 skilled nursing facilities (CDC, 2016). Transitions across levels of care have the potential to be costly and dangerous to these older adults (The Henry J. Kaiser Foundation, 2013). The Inspector General indicated that 33% of skilled nursing facility Medicare beneficiaries experience adverse events, with over 1/3 being related to medication errors (DHHS, 2014).Management of complex, medically ill older adults experiencing a transition from acute care to primary care require interprofessional teams to assess, treat, and develop comprehensive discharge plans for safe, quality care.
